Carmel
Carmel has over 39 years of nursing experience, holds a Diploma in Community Service (mental health), Diploma of Counselling, and Diploma of Management. In addition, she has also completed a practice certificate in Disaster Support and is a Nurse (AHPRA registered). She has gained extensive experience from working as a support coordinator for the past 7 years as well as previous roles in mental health nursing, case management experience from the Department of Corrective Service, Disability ACT and the Asbestos Response Taskforce. Carmel is dedicated to supporting Connections Case Management clients and goes above and beyond when advocating for their needs. Carmel is registered to provide both Level 2 and Level 3 support coordination
Omer
Vicki
Vicki has gained her business and administration experience from working in a government healthcare setting as an administration manager and a clinical scientist, she holds degree’s in Exercise Science and Sports Management, and has post graduate qualifications in Management and Leadership.
She has been working as a support coordinator for 3 years and is currently studying to be an Occupational Therapist at Charles Sturt University. She has participated in further professional development courses such as Trauma Training, Mental Health, Autism and SoSafe Training.
Vicki is passionate about helping others to live their best lives and building up their support systems.
Omer is an experienced and highly skilled support coordinator with a passion for helping people. With over 3 years of experience in disability, Omer has developed a deep understanding of the needs and challenges faced by individuals and families who require support services. He is dedicated to working collaboratively with clients and their families to develop personalised care plans that meet their unique needs and goals. Omer is skilled at identifying and accessing resources and services that can support individuals in achieving their desired outcomes. Omer is a lifelong learner who is committed to staying up-to-date with best practices and new developments in the field of support coordination. He regularly participates in professional development opportunities to enhance his knowledge and skills. Omer is dedicated to helping individuals and families access the services and supports they need to thrive.
Bianca
Bianca joined Connections Case Management in July 2023 as an experienced NDIS Support Coordinator and has been helping make a meaningful difference in participants lives ever since. She has a wealth of experience in the disability sector having previously been a NDIS coordinator in Canberra, a vocational training officer in a disability employment service in addition to completing multiple territory studies including: a bachelor’s degree in social studies, undergraduate certificates in counselling, mental health, and disability care services.
Bianca is passionate about building positive relationships with participants and their support systems to ensure that all clients are receiving the supports that they need to thrive. She always ensures that clients understand their NDIS plans, the NDIS scheme and looks to support clients with achieving their NDIS goals through utilising services of their choice.
Bianca has experience with a variety of disabilities including intellectual, physical and psychosocial and is passionate about treating all clients with a holistic and individualised approach.
Stef
Occupational Therapist
Stef is Connections Case Managements occupational therapist who graduated from La Trobe University, Victoria in 2008 with a Bachelor of Occupational Therapy. She has 15 years' experience working with clients from various populations including disability, mental health, drugs and alcohol, forensic, adults and youth. Stef is passionate about client centred practice and actively works with clients and their support systems to achieve outcomes. From her previous professional experience with Canberra Health Services, ACT Corrective Services, various private organisations and the NDIS, Stef is knowledgeable about different services and creative ways to help empower clients to reach their individual goals. She has a special interest in working with people with mental health issues and intellectual disability, with 6 years of experience working in this specialised area of practice.
Michelle
Meet Michelle, a compassionate Support Coordinator, and Psychosocial Recovery Coach with a wealth of experience and a heart dedicated to helping others thrive. With certifications in life coaching and mental health (Certificate IV), she blends professional expertise with personal insight, having navigated her own/family journey through mental health challenges.
Understanding that each person's path to wellness is unique, Michelle approaches her work with empathy and a deep appreciation for individuality. She believes in the power of personal agency, guiding clients to discover their strengths and chart their course toward recovery and fulfillment.
Beyond her professional pursuits, Michelle finds inspiration in the beauty of art, photography, and the great outdoors. Michelle cherishes moments spent with family and friends, recognising the importance of connection and shared experiences. Music serves as a source of solace and a means for expression, enriching her life and the lives of those she serves.
