Connections Case Management is a support coordination company operating in Canberra and regional areas, that aims to provide NDIS participants with a tailored service and exceptional level of support coordination. We offer Level 2 and Level 3 Support Coordination as well as Recovery coaching.

  • Support Coordination

    Our focus is making appropriate connections for our clients, be they mainstream or NDIS funded services. Here at Connections Case Management, we are dedicated to ensuring that participants are connected to local and quality providers allowing them to get the maximum benefits from their NDIS plans. We work with our participants to regularly review their NDIS plans and service providers to ensure they have the right people on their team.

  • Case Management

    We are available for private case management and have supported individuals in a variety of services, like accessing appropriate drug and alcohol rehabs, supporting clients to find appropriate aged care facilities, providers and connecting them with services to dispose with household items, and we have also supported families trying to get their family member on the NDIS. These are privately funded and a quote can be provided.

  • Corporate Mentoring

    Carmel is available for mentoring/coaching services for businesses that are interested in building the knowledge of their staff in general around the NDIS industry, and specifically support coordination and how to be an effective support coordinator. Carmel brings with her a wealth of experience in managing staff, teaching others how to be an effective time manager as well as how to be supportive to clients yet maintain good boundaries for a healthy working relationship.
